We may need to provide that information to our underwriters and those we appoint to assist us with the claim. We will not trade, rent, or sell your information. If you do not provide us with complete information, we cannot properly assess your claim.
You can check the personal information we hold about you at any time. If you provide us with personal information about anyone else, we rely on you to have informed them that you will provide their information to us, to whom we may provide it, the purposes for which we will use it, and that they can access it. If the information is sensitive, we rely on you to have obtained their consent on these matters.

Medical Authority and Declaration
- I understand that by investigating my claim or by accepting proof of my claim, Proclaim has made no acceptance of liability, nor waived any of its rights in defense of any claim arising under the policy.
- I declare that the information on this form and any attached documents is correct and complete, and that I have not withheld any information that could affect this claim.
- I agree to use my best endeavors and render all reasonable assistance and cooperation to Chubb in the assessment of my claim.
- I understand that my claim may be denied if the information supplied is untrue or I have not revealed all relevant facts.
- I authorize any hospital, physician, or other person who has attended me to furnish the claims manager, Proclaim, or its representatives with any and all information regarding any sickness or injury, medical history, consultation, prescriptions, or treatment, including copies of all hospital or medical reports.
- I authorize any employer, insurer, superannuation fund, or other organization or body through which I am or may claim similar benefits to furnish Proclaim with all information regarding coverage and claims for this sickness or injury to enable the assessment of my claim.
- I agree that a photocopy of this authorization shall be considered as effective as the original.
